  • Have your personal health information treated confidentiality
  • Express concerns and recommend changes to our policies without fear of reprisals, interference, coercion or discrimination
  • Have a clear explanation of the services you will receive and who will provide them
  • Accept of refuse service
  • Instructions on how to file a complaint with our organization or our assessor(s)

YOUR RESPONSIBILITIES

You have a responsibility to:

  • Treat Encompass Medical Experts Inc. staff or its service provider with courtesy and respect, free from discrimination and harassment
  • Provide consent so we can determine your current status and provide a report to the referring party
  • Provide complete and accurate information reasonably requested to allow medical professionals to assist you in determining your level of care needs
  • Tell us about any changes in your situation that could affect the outcome of your medical needs
  • Participate in your assessment
  • Prompt arrival for your appointment(s)
  • Inform us if you are unable to attend an appointment or if you will be late

CONFIDENTIALITY

In accordance with professional standards, all information is held confidential and released only through procedures consistent with the law and professional ethics. With a court order, records can be released without a client’s permission. Other exclusions to confidentiality may include reports of evidence of abuse, neglect or exploitation of children or endangered adults, specific threats against people, threats of suicide and other exclusions as provided by medical professional regulatory college policies and procedures.
